Mental Health Outreach Support Worker Responsible to Community Services Managers Hours of Work Various part-time opportunities available; maximum 26 hours, minimum 12 hours - Monday to Friday Responsible for The Mental Health Outreach Support Worker is as newly emerging role within the Age Well Teams, providing mental health support to, The position of Mental Health Outreach Support Worker will be working in partnership with the Primary Care Network's Age Well Teams. The role provides the opportunity to work innovatively with a range of partner organisations to enhance the experience of older adults living in care homes and their own home. The Age Well Teams provide early multi agency support for emerging needs, including Frailty clinics and care home teams and are working in partnership with Northamptonshire Mind to include mental health support for their patients. Experience of, or a real interest in, supporting older adults is essential. Working as part of our overall Integrated Holistic Support Team your primary responsibilities will be to:
- Support patients identified through the team who need mental health or emotional wellbeing support.
- Support patients through structured interventions, providing tools and strategies to support individuals' mental health and emotional wellbeing.
- Signpost patients to other 3rd sector services, support & other community resources that will benefit the patient and assist patients to access the services identified.
- Work with patients to build confidence to attend and engage in services and/or support identified.
- Review whether identified services and support are working for the patient.
- Maintain an individual case load whilst working cohesively alongside other Age Well partner organisations, supporting a collaborative approach.
- Support patients to enable them to remain as independent as possible within their own home and reduce the risk of crisis support.
- Participate in any other work identified by the team that is deemed suitable for a Mental Health Outreach Support Worker.
- Ensure all meetings/interventions with patients are recorded.
- Attend any meeting identified for the Mental Health Outreach Support Worker to attend, including Multi-Disciplinary Team meetings. Expectations: All staff are expected to embody our mission, values, and competencies. This includes the expectation that they will;
