The predicted salary is between 36000 - 60000 Β£ per year.

The post holder will be working in an integrated multi-professional patient flow team and will be involved in facilitating a safe and timely admission for all patients across community inpatients. The post holder will carry out assessing all incoming referrals to community inpatients and liaise directly with referring services including acute hospitals, community services, palliative services, GPs, etc. to ensure safe and timely admission into the units.

Responsibilities

  • Report to the Patient Flow Lead and independently assess patients being referred into the service and manage their own caseload as part of the team.
  • Deputise for the Flow Lead as required, including data collection and data reporting of admission and discharge information.
  • Provide support to junior colleagues and assist them in problem-solving issues relating to patient flow.
  • Facilitate admissions over the 7-day period to meet the needs of the service, including potential weekend and bank holiday work on a rota.
