About The Role
As an Ageing Well Coordinator, you’ll be a key member of a multidisciplinary team, working closely with GP surgeries and community partners within PCN. Your role will involve:
- Taking referrals from GP practices and arranging home visits
- Representing Northamptonshire Carers within the Ageing Well team
- Carrying out holistic assessments and patient observations
- Supporting people aged 65+ with complex long-term conditions
- Helping individuals avoid unnecessary hospital admissions or institutional care
- Promoting independence and wellbeing—helping people choose well, stay well, and live well in their own homes
About You
We’re looking for someone who is:
- Experienced in support work, wellbeing coordination or within health and social care
- Skilled in working collaboratively across multi-disciplinary teams
- Confident using electronic recording systems
- Knowledgeable about services available to support older people
- A great communicator—both written and verbal
- Empathetic, a good listener, and able to build trust with individuals and families
- Organised, self-motivated, and capable of managing a varied caseload independently
You may already be working as a primary care practitioner, within PCN primary care network or support worker.
