Overview

This is an excellent opportunity to gain experience of working within the Community Children\\\’s Nursing (CCN) Service based at Northampton General Hospital.

The post holder will be responsible for the management of a caseload of children/young people aged 0-18 years, assessing, planning and delivering evidence-based nursing care in the community setting across West Northants. You will work collaboratively with the child/young person, their parents/carers and the multi professional team around the family (including interagency working). Enhanced communication skills are required to promote good working relationships to facilitate the best outcomes for the child/young person. You will undertake lone working, risk assessments and adherence to local policy and procedures. You may deputise for the Lead Nurse in her absence, provide management in day-to-day running of the service, supervise junior team members and mentor students in training. The core service hours are 08:00-18:00, Monday to Friday, with contracted hours in a shift pattern of early 08:00-16:00 and late 10:00-18:00 across the working week. The role requires the ability to work flexibly across a 7-day service and may include unsocial hours and on-call duties, including End of Life care.

Responsibilities

  • Carry out a range of clinical skills in the community setting, e.g. enteral feeding, administration of intravenous therapies, injections, wound care, CVAD competencies.
  • Provide clinical expertise by setting standards in assessment, planning, implementation and evaluation of patient care needs using agreed protocols.
  • Provide clinical expertise in education and support for children and families in the community setting.
  • Support patients, families and colleagues in highly distressing or emotional circumstances.
  • Analyse and interpret presenting data and clinical presentations to meet complex needs, providing holistic care and ensuring timely referrals to other services as necessary.

  • Be aware of budgetary responsibilities and limitations; assist in managing resources within the directorate budget.
  • Maintain accurate and contemporaneous documentation across relevant systems.
  • Demonstrate professional standards of practice and behaviour in line with the Nursing and Midwifery Council Code; ensure systems support nurses in meeting their responsibilities.
  • Represent the Directorate at external meetings and working groups, disseminating outcomes within the organisation.

Quality Assurance, Policy and Service Development

  • Promote, monitor and maintain best practice in health, safety and security.
  • Manage adverse events and complaints per Trust Policy.
  • Support the department in developing high-quality, cost-effective specialist service through monitoring and quality initiatives.
  • Lead in creation and review of policies and guidelines to facilitate safe care for children, families and community nurses.
  • Support service feedback collection from users and inform service development.

Appraise, interpret and apply suggestions and directives to improve services and support the Lead Nurse in managing change.
