- Identification: A valid government-issued photo ID (e.g., driver’s license).
- Number of hours you are required to observe.
- University Advisor Name.
- University Advisor Email.
Mandatory Clearances
You must complete all background checks before entering a campus. Standard requirements include:
- State Criminal History Record: PA State Police Clearance Act 34 (Can say "VOLUNTEER or OTHER" purposes – No FEE for Volunteer).
- Child Abuse History Clearance: Act 151 Child Abuse Clearance (Can say "VOLUNTEER or OTHER" purposes – No FEE for Volunteer).
- Federal Fingerprinting: Act 114 FBI Fingerprinting (There is a fee).
- Health Screening: A negative TB test result dated within 3 months of the date the district receives it.
