About the job

Reablement plays a major part in reshaping the future of care and support – helping people to live longer, healthier and more independent lives in their own homes. It differs from traditional residential care. We support people to do things for themselves again, maintaining and improving independence and enabling more fulfilled lives at home. Reablement centres on recovery, supporting people with personal goals typically up to 6 weeks. You will work with service users to develop their skills and confidence to achieve daily independent living, enabling them to regain health and independence. Working with other colleagues such as occupational therapists and social workers, you will contribute to person-centred support plans and ongoing progress reviews.

Responsibilities

  • Provide personal care and support to residents, encouraging participation in a wide range of activities and promoting independence.
  • Support residents to take part in activities that enable independence and ensure their dignity and respect are preserved.
  • Develop and review person-centred support plans, working collaboratively with occupational therapists and social workers.
  • Attend progress reviews and contribute to the achievement of personal goals for each resident.
  • Maintain health and safety standards, record care provision accurately and support residents in maintaining their health and wellbeing.

Shift Patterns

Early shifts (approximately 07:30 am – 13:30 pm or 03:00 pm), late shifts (approximately 04:00 pm – 10:00 pm) and weekends are required.
