Are you ready to take the next step in your social care career? An exciting opportunity has arisen for an experienced and highly motivated professional to join the Craven Intermediate Care Hub Team in the role of Advanced Practitioner, on a full-time, permanent basis. The substantive working hours for this post are Monday to Thursday, 8:30am to 5:00pm, and Friday, 8:30am to 4:30pm.
The post holder will be required to work within a hospital setting, providing expert support to ensure safe, effective, and timely discharge planning. This position offers a significant opportunity to undertake a leadership role within the service, including deputising for the Team Manager as required. The successful candidate will contribute to the management and coordination of complex discharge pathways, working collaboratively with multidisciplinary partners to deliver high-quality, person-centred care and achieve optimal outcomes for individuals.
The Advanced Practitioner will provide professional leadership within the service, ensuring the promotion and maintenance of consistent, confident, and high-quality practice. The post holder will hold formal responsibility for the supervision and professional oversight of Social Care Coordinators, supporting effective practice, adherence to statutory requirements, and the delivery of safe and appropriate interventions.
About the Role:
As an Advanced Practitioner, you will play a pivotal role in shaping the future of our Craven Intermediate Care Service. You will:
- Deputise for the Team Manager, supporting the operational delivery and development of the team.
- Lead on reflective practice, mentoring and coaching colleagues to build confidence and consistency in practice.
- Streamline processes, helping to improve the efficiency and quality of our assessments and outcomes.
- Continue to undertake Intermediate care pathway work in support of the person using the service.
This role is ideal for someone who is ready to step up, take on more responsibility, and develop their leadership skills in a supportive and forward-thinking environment.
