At 54 Reasons, our Family Integrated Case Management Senior Practitioners play a vital role in strengthening families and helping children thrive. This role goes beyond traditional case management – it focuses on early intervention, building strong relationships with families, schools, and community services to provide the right support at the right time. By working collaboratively and taking a family-centred approach, F.I.C.Ms help reduce risk, improve family wellbeing, and create safer, more stable environments for children.

In this (full-time, max term) Family Integrated Case Management until December 2026, you’ll work as a part of our 54 Reasons Family & Child Connect team in Mount Isa to help us create lasting change for children everywhere. You will make a difference by:
- Building strong, trusting relationships with families to identify strengths, needs, and goals, and providing holistic, family-centred case management support.
- Conducting assessments and developing tailored support plans that promote child safety, wellbeing, and positive family outcomes.
- Working collaboratively with schools, community organisations, and service providers to coordinate support and ensure families are connected to the right services.
- Providing early intervention and practical support to families to reduce risk factors and strengthen family functioning.
- Maintaining accurate case notes, documentation, and reporting in line with organisational policies, procedures, and child safeguarding requirements.
Your deep expertise in Family Integrated Case Management will be invaluable. You’re a compassionate and skilled practitioner who builds trust with families and works collaboratively with schools and community services to create meaningful change. Your ability to listen, assess family needs, and provide practical, strengths-based support enables families to feel empowered, supported, and connected to the services they need.
