We have an exciting opportunity for a Work and Health Coach to join the North West Leicestershire GP Federation to support the Primary Care Network, as part of our WorkWell Programme team. Work and Health Coaches are supported and mentored by our Senior Work and Health Coach and Leadership Team, and work as part of a multi-disciplinary team in a patient-facing environment. The post holder will be employed by North West Leicestershire Federation and this position would be based at Whitwick Health Centre.
Main duties of the job
- You will be responsible for assessment and specialist employment advice, support and interventions.
- You will manage a caseload of people assessing individual barriers into employment, experienced through physical health, mental health and social situation.
- Using the WorkWell pathway work plan, you will prescribe either multi-disciplinary in-house support or triage/signpost, refer to and collaborate with the care navigation team.
- You will facilitate clients being referred to the service quickly so that they do not remain unsupported whilst on sick leave and support them to return to work swiftly (where appropriate), or to follow the destination that is best suited to their circumstances, which enables individuals to achieve positive service experience outcomes.

The North West Leicestershire GP Federation consists of 12 GP Practices providing care to approximately 116,000 patients. The GP Federation operates the North West Leicestershire Primary Care Network which is the largest within Leicester, Leicestershire and Rutland. We want to support the development of the team and provide additional capacity as the service grows. We are an ambitious and innovative organisation looking to develop services for our patients and provide opportunities for our staff to learn and grow. The WorkWell programme team is made up of the Senior Work and Health Coach and two Work and Health Coaches. This team operates within the larger Social Prescribing Link Worker team. We are a forward thinking Primary Care Network that is committed to working as a multidisciplinary team and recognise and utilise the full potential of every individual.
We are looking for a full time or part time (minimum 22.5 hours) individual to join our team and can facilitate flexible working and accommodate some home working. We are also happy to consider the role on a part time basis. Experience
- An ability to initiate and develop relationships with partners including local clinicians, employers and Jobcentre Plus staff.
- An ability to work independently, reliably and consistently.
- Good report writing skills.
- Good numeracy and information management skills.
- Working with disabled people and individuals with health conditions who may also experience complex barriers to starting, succeeding or staying in work.
- Vocational assessment and profiling skills.
- A good understanding of the health and social care environment and roles and responsibilities within it.
- Understanding of the background to and aims of current healthcare policy and appreciate the implications of this in engagement.
- Experience of supporting people in a holistic and patient-led way.
- Experience of supporting people through VCSE and collaborative working.
Qualifications
- Degree level or equivalent level of experience.
- Evidence of continuing professional development.
- Coaching qualification.
