Responsibilities
- Provide ongoing, person-centred support to customers with profound and enduring learning disability and mental health conditions to improve quality of life and promote independent living.
- Deliver a structured, flexible community outreach service tailored to individual needs.
- Support customers to sustain their tenancies, including understanding tenancy responsibilities such as property care and rent payments.
- Create and maintain a safe, supportive, and empowering environment for customers.
- Work closely with customers to build confidence, independence, and community integration.
- Liaise effectively with statutory and voluntary agencies, including Mental Health Teams and Adult Social Care, to deliver coordinated support.
- Contribute to the development of services to reflect changing customer needs and support long-term positive outcomes.
- Experience supporting vulnerable people, with a clear understanding of professional boundaries and diverse backgrounds, as well as good understanding of safeguarding.
- Ability to deliver structured, person-centred support, including needs assessments, support planning, and risk assessments.
- Strong communication skills, with the ability to maintain clear, accurate records and reports using Microsoft Office and digital systems.
- Good administrative skills, with strong attention to detail and the ability to manage service user information and meet targets.
- A positive, resilient, and inclusive approach, demonstrating confidence in challenging situations and a commitment to equality, diversity, and inclusion.
- A full UK driving license is essential for this role.
- This position requires an Enhanced DBS check (with Barred Lists).
