North & South Gloucestershire Primary Care Network is seeking a Social Prescriber to join their Living Well Team. The role focuses on connecting individuals to community-based support to enhance their wellbeing, addressing factors such as social isolation and emotional needs.
Located primarily at Churchdown Surgery in Gloucester, the position offers diverse responsibilities, including home visits and liaising with various community groups. Candidates should be able to work flexibly and possess their own transport.
This role does not qualify for UK visa sponsorship.
