We are seeking a dedicated and experienced Social Worker to join our South Adult Locality Team. This well-established and supportive Adult Social Care Team is based at Town Hall, Weston-super-Mare and offers hybrid working, covering Weston, Worle and the surrounding villages.
In this role you will work within key legislative frameworks including the Care Act 2014, the Mental Capacity Act 2005 and the Mental Health Act 1983, making defensible decisions in partnership with people with care and support needs and supporting them to take positive risks to promote and balance their independence, safety and wellbeing. You will undertake Care Act Assessments, co-produce care and support plans with individuals, identify their strengths and assets to determine person-centred outcomes, undertake Mental Capacity Assessments and be involved in Best Interest Decision making. You will hold a manageable caseload and share responsibility for coordinating safeguarding enquiries in a person-centred way.
Key Responsibilities- Undertake Care Act assessments, reviews, care and support planning in line with statutory guidance
- Apply the Mental Capacity Act and contribute to Best Interest decisions and Safeguarding Interventions
- Work collaboratively with health colleagues, providers and other agencies to deliver coordinated responses to complex needs
- Ensure compliance with all relevant policies, procedures and statutory frameworks
This position is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check. A valid driving licence and own transport, or the ability to provide an alternative suitable method of travel, is an essential condition for this role.
Essential Criteria- Qualified Social Worker registered with Social Work England
- Demonstrable experience in statutory adult social care, particularly within learning disability services
- Strong working knowledge of the Care Act, Mental Capacity Act, Human Rights Act and safeguarding legislation
- Proven ability to manage complex caseloads and make sound, defensible decisions
- Effective partnership working with multi-disciplinary professionals, individuals and families
- Previous Social Work Practice
- Ability to prioritise tasks and work under pressure
