Pcn Clinical Pharmacist @ Churchmere Medical Group in Manchester

You'll be part of the practice-based pharmacy team at Churchmere Medical Group, embedded in the practice team, known to your GPs and your patients, and taking genuine clinical ownership of the medicines work that keeps a busy general practice running safely and well. You’ll join an established team with two PCN pharmacists already supporting Churchmere, largely remotely, alongside pharmacy technicians and dedicated medicines admin support. Your role is different: a named practice base, on site and face to face with patients and colleagues. You'll work in close partnership with the wider pharmacy team, the GPs and the whole practice team to deliver better, safer outcomes for patients. Your clinical knowledge and judgement will be used every day and expected every day. Where you hold an independent prescribing qualification, you'll use it. Where you're working towards one, we'll support you to get there. You'll be employed by North Shropshire PCN and supported by a leadership team that includes a pharmacist Clinical Director. You'll also be part of a wider PCN pharmacy team of eight pharmacists and six pharmacy technicians.

Salary: £47,220 - £59,814, dependent on experience and prescribing status. Full time preferred; part time considered.

Responsibilities

• Repeat Prescribing & Medicines Optimisation: Clinically review repeat prescriptions, optimising therapy, deprescribing where appropriate and improving patient safety. Independently prescribe where qualified.
• Medicines Reconciliation: Reconcile medicines after hospital discharge and other transfers of care, resolving discrepancies and reducing medicines‑related harm.
• Structured Medication Reviews: Review patients with polypharmacy and multimorbidity, optimising treatment, addressing monitoring needs and implementing prescribing changes where appropriate.
• Long‑Term Condition Management: Manage clinics for hypertension, diabetes, COPD, heart failure and anticoagulation, improving outcomes and reducing avoidable GP appointments and admissions.
• Medicines Safety: Lead MHRA alerts, ECLIPSE monitoring, high‑risk drug audits and risk stratification.
• Patient‑Facing Medicines Support: Provide medicines advice to patients, improving understanding and reducing GP workload.
• Care Home Medicines Support: Review medicines, support ordering processes and advise care home staff.
• QOF & Enhanced Services: Support proactive QOF delivery through medicines optimisation and patient reviews.
• Antimicrobial Stewardship & Clinical Guidance: Advise the practice team on medicines, support antimicrobial stewardship and implement local and national prescribing guidance.
• Independent Prescribing: If you hold an independent prescribing qualification, you will use it fully; if you are working towards it, we will actively support you to complete it.

Person Specification

Essential

• Registered with the General Pharmaceutical Council
• MPharm degree or equivalent
• Excellent clinical knowledge across common acute and long‑term conditions seen in general practice
• Strong interpersonal and communication skills, able to work confidently with patients, GPs and the wider practice team
• Ability to manage own caseload, prioritise effectively and work under pressure
• Commitment to working at the top of your clinical capability and developing your practice
• Full driving licence and own transport

Desirable

• Independent prescribing qualification (or actively working towards)
• Completion of CPPE Primary Care Education Pathway (or willingness to undertake)
• Experience in a primary care or general practice setting
