North Sedgemoor PCN is seeking an enthusiastic and committed Salaried GP to join our friendly, progressive team working across Brent Area Medical Centre, Axbridge Medical Centre and Cheddar Medical Centre. About us
North Sedgemoor PCN brings together GP practices caring for around 50,000 people across Burnham on Sea, Brent, Highbridge, Cheddar and Axbridge. We are clinically led, collaborative and community focused. Our team includes GPs, Occupational Therapists, Pharmacists, Care Coordinators, Health Coach and an experienced Paramedic who will be a peer and mentor for this role. We are a learning organisation. You will have a named supervisor, regular clinical forums and protected time for development. We value curiosity, high standards and kindness. Ideas are welcomed and improvement is part of daily work. This is an exciting time to join us. We are scaling services and modernising how we deliver urgent and planned care in practices, in people's homes and in care homes. You will be supported by clear pathways, friendly administrators and a welcome team who help new colleagues settle in. We offer funded training aligned to your scope of practice, access to First Contact Practitioner routes, Masters level modules and Independent Prescribing where appropriate. You will also have NHS pension if eligible, generous annual leave and access to wellbeing resources.
Job responsibilities
- Clinical Care: Assessing, diagnosing, and managing complex, frail, or palliative patients.
- Case Management: Overseeing care for patients with multiple long-term conditions (frailty/multimorbidity).
- Multidisciplinary Team (MDT) Leadership: Leading MDT meetings, engaging with community services, social care, and specialists.
- Proactive Care Planning: Creating individualized care plans, focusing on reducing unnecessary hospitalizations and managing palliative needs.
- Structured Medication Reviews: Undertaking in-depth reviews to optimize medication for patients with complex needs.
- Supporting Care Coordination: Working alongside care coordinators and navigators to ensure smooth, integrated care across primary/secondary care boundaries.
