North Sedgemoor Primary Care Network is based in North Sedgemoor supporting four GP practices across a patient population of approximately 50,000. This innovative forward-thinking PCN has decided to work together collaboratively with Somerset Foundation Trust to improve the quality of services and provision for their patients.

The Neighbourhood Transformation Manager will work alongside the PCN Manager, Clinical Director and Neighbourhood Development Manager to achieve success in line with the PCN Network Contract DES and wider neighbourhood priorities. The post holder will provide senior strategic leadership for the design, mobilisation, implementation and evaluation of North Sedgemoor neighbourhood transformation, with an initial portfolio focus on frailty and joined-up care.

The postholder is accountable through the agreed PCN and Somerset NHS Foundation Trust governance arrangements for translating neighbourhood strategy into sustainable integrated services, operating models, pathways, workforce arrangements, performance measures and improvement programmes across organisational and professional boundaries. Working with PCN, NHS, local authority, voluntary, community and social enterprise partners, the postholder will exercise a high degree of autonomy in leading complex transformation, resolving barriers to implementation and providing assurance regarding delivery, quality, risk, resources, benefits and population outcomes.

Main duties of the job:

  • Lead a portfolio of interdependent neighbourhood transformation, with initial responsibility for frailty and joined-up care and the flexibility to lead further agreed neighbourhood priorities.
  • The post holder's immediate priority would be to translate the emerging neighbourhood frailty model into reliable day-to-day delivery across primary care, community services and wider neighbourhood partners.
  • Enable PCN, SFT, local authority, VCSE and other neighbourhood partners to work as one neighbourhood system to design, implement, measure and continuously improve a proactive, personalised model of care for people living with moderate and severe frailty.
  • Create mechanisms for local people and communities to contribute to and shape the design and delivery of local services.

We offer a supportive, professional environment where you will be able to develop your skills further, share learning with colleagues, and play a key role in shaping the quality of prescribing across the network. You will also benefit from NHS pension, generous annual leave, wellbeing resources and the opportunity to work in a team where your contribution is recognised and valued.
