North Lincolnshire Home First Community Support is a registered Rehabilitation and Reablement service that provides time limited intervention and therapies to people within their own homes. We strive to deliver support that puts people at the centre of our services. Our aim is to ensure that a person can remain at home, feeling confident and safe to live as independently as possible.
Rapid Response Rehabilitation worker post holders provide rehabilitation support to individuals in the community following an accident, event or illness, supporting to regain and maintain their independence to their optimum potential. This may involve following therapy plans and working in conjunction with other health and social care professionals.
The role also involves direct work such as supporting with moving and handling, attending to personal care needs and administering medication as well as maximising opportunities for social inclusion and emotional wellbeing. Rapid Response Rehabilitation workers also respond to crisis visits, as well as providing welfare visits in a person’s home, to support and identify if further support is required after an event and implement required actions accordingly.
The ability to work flexibly including weekends and unsocial hours, including bank holidays, and be able to travel around North Lincolnshire is essential. The role offers a fantastic work life balance through a 25 hour per week contract and as a Rapid Response rehabilitation worker you will be part of a fast paced and developing service that has opportunities for further development and progression.
