About the Role
We are looking for a dedicated Family Support Worker to join our vibrant Family Hub team. You will play a vital role in supporting families with children aged 0β19 (or up to 25 with SEND), helping them navigate challenges and build resilience.
Key Responsibilities
- Provide one-to-one and group support to families facing a range of issues including parenting, housing, health, and education.
- Work collaboratively with professionals from health, education, and social care to deliver integrated support.
- Conduct home visits and assessments to identify needs and develop tailored support plans.
