About the Role

We are recruiting three Practitioners to join our Adult Support Services team. These roles offer a mix of office-based, community, and hybrid working. Posts may be based primarily in Medway or across Dartford, Gravesham and Swanley (DGS), depending on service requirements. The role involves delivering mental health and wellbeing support to adults, helping individuals improve their wellbeing, build resilience and access appropriate services. Delivery includes a combination of:

  • one-to-one support
  • group-based interventions
  • client engagement and assessment

The balance of delivery will vary depending on operational need and service demand.

What You Will Be Doing

  • Managing a caseload and delivering structured mental health support
  • Undertaking assessments and engaging individuals into services
  • Delivering one-to-one and group-based interventions
  • Supporting individuals to identify goals and progress their recovery
  • Identifying and managing risk, including safeguarding concerns
  • Maintaining accurate records and contributing to service outcomes
  • Working collaboratively with colleagues and partner organisations

The role will be based at one of North Kent Mind’s main offices (Medway or Dartford), with service delivery also taking place across various community locations in North Kent. Office environments differ between locations and may include a combination of open-plan and smaller room-based layouts, with varying levels of noise, lighting and foot traffic. The primary work base and delivery pattern will depend on service requirements and may vary over time, including across Medway and DGS locations. This will be discussed as part of the recruitment process.   • Have experience or understanding of supporting people with mental health needs

  • Are able to deliver structured support (one-to-one and/or group-based)

  • Can work both independently and as part of a team
  • Are committed to delivering person-centred and recovery-focused support
