In January 2026, North Hampshire Urgent Care introduced a new primary care service within the North Hampshire area. This service will provide a daytime Paramedic and ACP-led home visiting service for patients within four Primary Care Networks (PCNs) presenting with unplanned acute and subacute primary and urgent care needs. We are therefore looking to add to our team of experienced Paramedics (either with existing post-registration qualifications of urgent care, or those with a strong interest in primary/urgent care who would be interested in developing their career further in this area). The service operates 10:00-18:00 Monday to Fridays (excluding bank holidays) and we would be looking for someone to be contracted to every Wednesday, Thursday and Friday 10:00 - 18:00. Please note, this is a fixed term position.
The successful applicant will be required to demonstrate the NHUC Values, have good organisational skills, and be competent in using IT tools and systems. They must be able to manage changing demands and priorities and work effectively as both part of a team and independently. Experience of working in a similar environment would be beneficial but not essential.
The successful candidate will be autonomously assessing, diagnosing and discharging or referring patients within their scope of practice with the support of GP practice colleagues and remote support provided by NHUC clinicians as required. We are looking for highly motivated and enthusiastic individuals who will help support the new service and build relationships with the practices and PCNs that the service is operating within as part of the role.

Job Responsibilities
Role Summary:
- To work as part of the primary care home visiting team, providing acute assessment, investigations and diagnostics of unplanned acute and subacute urgent and primary care conditions to housebound patients.
- The post holder will be responsible for providing appropriate urgent primary care level treatment and either discharging or onward referring patients as appropriate within their scope of practice with the support of remote clinical supervision.
- To deliver high quality safe, effective patient centred care.

- To integrate pre-existing Paramedic knowledge and skills into the Same Day Care service, providing first contact patient care.
Main Duties and Responsibilities
- Clinical Care: Autonomously undertake visits to patients' home addresses, providing assessment and diagnostics as required of acute and subacute primary and urgent care presentations. Undertake holistic assessment of wider care needs as appropriate. Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols. Work within scope of practice, escalating to colleagues as required within agreed protocols. Ensure that patients receive high quality clinical care, delivered in a timely manner. Ensure complete and accurate documentation of each and every patient contact. Work as part of multidisciplinary and multiagency teams in order to ensure patients' needs are met.
- Professional: Adhere to the HCPC Standards of Conduct and Standards of Proficiency for Paramedics. Ensure that professional practice adheres to organisational policies, procedures and guidelines. Maintain a professional manner. Maintain confidentiality with regard to information pertaining to patients and staff. Ensure that the service interfaces with all other stakeholders in a professional and productive manner, providing an effective service to partner organisations and other service providers. Act as a positive role model. Develop a professional partnership relationship with the GP surgeries utilising the home visiting service.
- Communication: Communicate effectively with patients and families regarding complex information about their care. Provide advice and information to patients, carers and their families where appropriate. Communicate effectively and work collaboratively with medical, nursing and allied health professionals to ensure delivery of a coordinated service. Ensure accurate documentation record keeping in line with local policies and HCPC guidelines. Promote and demonstrate effective communication networks within the organisation, with partner organisations and other service providers.
- Clinical Governance: Cooperate with all investigations and audits as required. Adhere to North Hampshire Urgent Care's policies, procedures and guidelines.
- Service Improvement: Contribute to quality improvement within the service through the identification and initiation of changes, which lead to better evidence based practice and improved standards of care. Participate in clinical audit identifying areas of best practice and areas for improvement. Contribute to the setting and monitoring of standards of care. Help develop the services available to the urgent care population to reflect identified needs.
- Educational/Personal Development: Contribute to the development and maintenance of an excellent learning environment. Support clinical practice and development of knowledge and skills. Adhere to professional registration requirements to maintain professional development, participate in supervision and in line with HCPC guidance. Participate in defining own developmental plan to maintain and develop own clinical skills and ensure own professional development. Undertake and complete all statutory and mandatory training, in particular for Basic Life Support or Intermediate Life Support, Information Governance, Infection Prevention Control, Safeguarding and conflict resolution. To access regular peer support and clinical supervision. Contribute to your own personal development and participate in an appraisal and regular performance reviews. To support and coach new colleagues.
- Other: Demonstrate commitment to flexible working patterns, to meet the needs of the service and staff. Operate NHUC vehicles in a professional manner at all times, being aware the post holder is visibly representing NHUC at all times while in use. Maintain vehicle standards of cleanliness.
Person Specification
- Skills: Ability to assess and manage patient risk effectively and safely. Excellent interpersonal, verbal and written communications skills. Time management and ability to prioritise workload. Experience of EMIS Web.
- Knowledge: Awareness of policy developments related to the delivery of primary care services including General Practice, Urgent Care, Clinical Governance, Quality & Outcomes Framework. Understanding of evidence based practice including knowledge of national standards that inform practice (e.g. National Service Frameworks, NICE guidelines etc.). Understanding of their accountability arising from the HCPC Standards of Conduct and Standards of Proficiency - Paramedics. Good knowledge of Safeguarding both Adults & Children at Level 3.
- Qualities & Attributes: An understanding, acceptance and adherence to the need for strict confidentiality. Ability to use own judgement, resourcefulness and common sense. Ability to work without direct supervision and determine own workload priorities. Ability to work as part of an integrated multi-skilled team. Able to work under pressure. Able to work in a changing environment. Able to use own initiative.

Qualifications:
- HCPC Registered Paramedic.
- Full clean UK Driving Licence.
Experience:
- At least two years clinical experience working operationally in either a primary, urgent or emergency care setting.
- Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge.
- Experience of autonomously assessing, diagnosing and managing a wide range of acute and subacute presentations.
- Evidence of working autonomously and as part of a team.
- Proven ability to evaluate the safety and effectiveness of their own clinical practice.
- Experience of working in a Primary Care, Community, Triage and/or A&E setting.
- Experience of engaging external stakeholders.
