North Halifax PCN is seeking a Care Co-ordinator to join our Personalised Care Team on a part-time basis, combining patient-facing reception duties with care coordination and improvement projects.
The role is 25 hours per week, based at Mixenden Community Hub with occasional travel to member GP practices. You will support the PCN DES and wider service delivery.
