Job summary
We are looking for an experienced Advanced Practitioner (Nurse, Pharmacist, Paramedic, or other AHP) to join North Halifax PCN.
This role supports multiple practices, enhances services, and involves autonomous care.
Key Responsibilities
- Conduct full clinical assessments, diagnosis and independent prescribing.
- Manage acute and chronic conditions using evidence‑based practice.
- Provide holistic, patient‑centred care.
- Support advance care planning, admission avoidance and improve health‑promotion strategies.
- Lead and contribute to clinical education, supervision and team development.
- Collaborate across the PCN and wider healthcare system.
- Prioritise and manage complex caseloads including frailty and long‑term conditions.
- Participate in audit, service improvement and clinical governance activities.
- Maintain accurate, contemporaneous patient records.
- Operate within the Advanced Practice Competency framework, producing an annual portfolio with the clinical supervisor.
- Adhere to professional codes of conduct (NMC, HCPC) and work within own competence.
Person Specification – Experience
- Essential: Knowledge of clinical governance framework; ability to write and present information effectively; broad knowledge of professional and healthcare policy; experience of advanced clinical skill development.
- Desirable: Knowledge of Systm One & EMIS.

- Essential: Registered with NMC or HCPC with advanced clinical practice / Advanced Nurse Practitioner qualification; independent prescriber; evidence of post‑registration study; ability to undertake full clinical assessments and prescribe autonomously; management of complex frailty and long‑term conditions.
- Desirable: Experience of clinical and educational programmes; relevant postgraduate education (PG Cert Clinical Education, Master’s in advanced clinical practice); leadership in enhanced services and PCN development; participation in care planning, audit and continuous improvement.

- UK registration with current professional registration.

- Ability to work in a multidisciplinary team and collaborate with the PCN Manager, Clinical Director, and other AHPs.
