North East & North Cumbria ICB is seeking an experienced Band 6 All Age Continuing Care Nurse to join our All Age Continuing Care service in County Durham and Middlesbrough. You will support patients with complex health needs, ensuring safe, effective and appropriate packages of care in line with the National Framework for NHS Continuing Healthcare and Complex Care Pathways.

You will work autonomously as a Nurse Assessor / Case Officer, coordinating holistic assessments and contributing to.
