North East Derbyshire Primary Care Network Ltd is looking for a Community GP to join our dynamic and supportive Home Visiting Team. This is your opportunity to deliver high-quality, holistic care to housebound patients and those in residential or care settings while shaping the future of community healthcare.
What’s on Offer?
- 2‑3 days/week (Mon & Thurs required, 09:30‑14:30; flexible third day)

Main Duties
What you'll do:
- Provide clinical leadership and supervision to the Home Visiting Team
- Conduct home visits for complex cases, including end‑of‑life care
- Lead senior clinical triage and support the Local Navigation Hub
- Work collaboratively across social care, ambulance, mental health, and community services
- Deliver advanced care planning and reduce avoidable hospital admissions
- Contribute to teaching, mentoring, and quality improvement

- Barlborough Medical Practice
- Killamarsh Medical Practice
- The Springs Medical Partnership
- The Valleys Medical Partnership
Serving over 40,000 patients, we are large enough to innovate and small enough to stay personal. All practices are close to the M1, with easy access from Chesterfield, Sheffield, Derby, and Nottingham.
Applications may close early if sufficient interest is received. Tier 2 sponsorship not available.
Person Specification
Experience:
- Demonstrable interest and experience in elderly care
- Understanding of adult safeguarding and Deprivation of Liberty Safeguards (DoLS)
- Experience in multidisciplinary team working and senior clinical triage
- Experience with Comprehensive Geriatric Assessment (CGA)
- Experience in clinical supervision or mentoring
- Familiarity with SystmOne clinical system
Qualifications:
- GMC registration with a licence to practise
- Inclusion on the NHS England GP Performers List
- Vocational training in General Practice
- MRCGP
- Postgraduate qualification in Geriatric Medicine or Care of the Elderly
- Qualification in medical education or clinical supervision
- Evidence of leadership development
Skills and Attributes:

- Excellent communication and organisational abilities
- Ability to work across professional and organisational boundaries
