Main duties of the job:
- Work within the PCN multidisciplinary team (including GPs, pharmacists, paramedics, care coordinators etc) to support patients to improve their health and wellbeing by connecting them to non‑medical community‑based services.
- Provide personalised support to individuals, families and carers to take control of their health and wellbeing, with a focus on supporting patients with complex social needs, long‑term conditions, and those at risk of health inequalities.
- Take referrals from Core Network Practices and a wide range of agencies and manage these through to completion.
- Use a holistic approach based on what matters to the person and the wider determinants of health, and co‑produce a simple personalised care and support plan with patients.
Requirements:
- A full UK driving licence and access to a vehicle with business insurance is essential, as the role involves travelling across the PCN area.
