Position Overview

The Transition Lead at River Tees Hospital School plays a central role in preparing learners for adulthood by supporting successful transitions into, through and beyond their time at the hospital school. Working closely with learners, families, schools and multi-disciplinary professionals, the postholder will coordinate personalised transition planning that promotes independence, resilience and aspiration. A strong emphasis is placed on developing learners’ readiness for future education, training, employment and independent living, while ensuring continuity of learning and emotional wellbeing. The role is underpinned by a commitment to inclusion and safeguarding, enabling learners to re-engage with education and make positive, informed choices about their next steps and longer-term outcomes.
