Our adult access team is seeking a post-ASYE social worker to support the Hospital at Home initiative. This innovative, preventative model of care helps adults remain safely at home with appropriate support, reducing unnecessary hospital admissions and promoting independence.
The Access Team plays a key role by coordinating assessments, care planning, and support delivery. We offer a hybrid working model to help manage work-life pressures and have flexible hours to support your personal needs.
