Social Work Support Assistant – Adult Social Care

Quarterly vacancies: 2 Permanent, Full Time (37 hours)
Location: Care & Support Team based in Eddie Ferguson House, Blyth

About the role

As a key worker for adults with care needs over 18 years of age, you will be the main point of contact for support and advice to them and their families, ensuring their safety and helping them live as independently as possible. You will personally complete needs assessments, considering safeguarding, reviewing continuing healthcare needs and promoting self‑directed support. Working with social workers, care managers, GP practices and community health services you will empower service users to live independently, manage risk and protect them from harm.

The main responsibilities include

  • Carrying out assessments and collaborating with service users to design solutions that meet their desired outcomes, seeking guidance where needed.
  • Managing and planning such work effectively to monitor and review outcomes.
  • Undertaking Carers\’ Assessments as part of the care management process.
  • Carrying out Risk Assessments as part of the care management process.
  • Participating in the duty system to pick up ad‑hoc work and acting on information under supervision.
  • Shadowing students and newly appointed staff from other disciplines.
  • Participating in discharge planning (from hospital) and care planning meetings.
  • Ensuring all written and electronic records are maintained to organisational standards.
  • Providing written reports, outcomes of assessments and recommendations.
  • Advocating on behalf of service users.

Essential requirements

  • NVQ Level 3 in social care or equivalent.
  • Experience working with adults in a social care setting.
  • Knowledge of legislation relevant to mental health and adult social care.
  • Experience working with people experiencing mental illness, learning disability or physical conditions in adults or older people.
  • Valid driving licence and access to a car for work purposes.
