We are looking to appoint a temporary Care Facilitator to be responsible for negotiating and arranging appropriate packages of care for individual service users as specified by Social Workers/Care Coordinators/Trusted Assessors. You will be a member of the Care Facilitator and Direct Payments Team within Adults and Health Services. This can be a high-pressure position so the successful applicant will need to be good at prioritising and be able to work well as a team and individually when under pressure.
WHAT WILL I NEED?
- Required Qualifications: 5 GCSEs including English Language (9-4/A-C grades) or equivalent relevant qualification.
- NVQ/Apprenticeship level 2 in Health and Social Care, or equivalent qualification.
- Excellent ICT skills.
