We currently have a fantastic opportunity for a Co-ordinator to provide valuable support for people in our vibrant Extra Care schemes.

Our Extra Care schemes offer residents a clear focus on wellbeing, promoting care and lifestyle opportunities that truly do create ‘better lives for older people’. We are committed to offering fulfilling and worthwhile careers to all our staff – enabling you to deliver the quality of service we promise.

You will work with the Extra Care Registered Manager and Shift Leaders across both of the Local Authority run Extra Care schemes at Clasper Court and Hagan Hall to ensure that the highest standards of care are provided. You will also work in conjunction with people, families and friends to promote a positive atmosphere within the service.

Key elements of the role:

  • Identify the needs of residents through reading and contributing to individual life stories
  • Creation of person centred support plans
  • Develop and maintain positive relationships with all residents, family, visitors and colleagues
  • Rota planning and supervision
  • Co-ordinate transport to support day visitors to attend the scheme
