Senior Community Nurse (Workington ICC)

The Workington Community Nursing Team have an exciting vacancy for a 6-month fixed term band 6 Senior community Nurse, for 37.5 hours per week.
We would welcome applicants from nurses within community nursing, or from acute nursing backgrounds, who are seeking to develop their skills within a senior community nursing role. We would look to support a motivated and enthusiastic nurse to take on this role. Full preceptorship will be offered as part of this career progression.
The role involves the delivery of holistic, patient-centred care in the community as well as complex caseload management. The successful applicant will be expected to manage the day to day running of the service and allocation of workload. They will also be required to be the \'triage nurse\' on a rota system. They will be expected to provide highly skilled evidence based nursing interventions to patients within a community setting.
Main duties
You must be able to demonstrate the ability to work within a changing environment and have good communication skills. The successful applicant must be able to demonstrate the ability to work on their own, and also as part of a team. You will be supported by the Community Nursing Team Leads and fellow Band 6 Senior Community Nurses to deliver nursing care covering weekdays, weekends, bank holidays and evenings.

This role requires you to demonstrate the skills and knowledge to support the provision of high-quality care, to support patients with complex needs within the community setting, and to support the Team Lead to provide clinical leadership and operational management of the community nurse team along with management of the clinical caseload for the team. Community experience is therefore desirable.
This is an excellent opportunity for dedicated and committed registered nurses who are looking to develop their clinical skills and expertise in caring for patients, working closely with health and social care colleagues to prevent hospital admission, facilitate hospital discharge and promote independence.

Person Specification
Qualifications
- Level 1 registered nursing qualification
- Evidence of continuous professional development to degree level or equivalent experience
- Post Registration qualification in Acute and Chronic disease management
- Nurse Prescribing
- Specialist qualification in Community Nursing or equivalent
Experience
