Rehabilitation Assistant

An exciting opportunity has arisen for a Band 3 rehabilitation assistant to join Workington ICC. The successful candidate will be based with the Workington Community Rehabilitation team.

We are looking for a motivated and caring individual, preferably with some community experience, but more importantly someone who is enthusiastic about rehabilitation with older people, providing support to people with long‑term needs and who enjoys working in a team.

The Community Rehabilitation Team is a friendly multidisciplinary team, with staff who work together to facilitate early discharge from hospital or prevent unnecessary admission.

Main duties of the job

A rehabilitation assistant in a multi‑disciplinary team is expected to work with other health and social care professionals to develop the ethos of multi‑disciplinary, integrated care and seamless services ensuring that patient care is delivered to the highest possible standard. As a rehabilitation assistant reporting to a registered health professional, the post holder will implement rehabilitation plans for patients in the community and other settings, observe the patient\’s condition, providing direct care with a rehabilitation and therapy focus. This may include supporting patients with personal hygiene and activities of daily living.

Flexible working will be required to meet the needs of the patient group.

The ability to work over 7 days is a requirement of the post. The hours of work will be on a rota covering a shift pattern between the hours of 8:00 a.m. and 6:30 p.m.

Continuing professional development is actively encouraged and as part of the team you will have the opportunity to take part in the process.

Person Specification

Skills and Aptitudes

  • Competency to provide clinical observations and basic diagnostics
  • Ability to apply practical skills to maintain patient independence
  • Able to use initiative, maintain confidentiality, exercise individual responsibility and judgement.
  • Basic IT skills.
  • Able to communicate tactfully and persuasively with colleagues, patients and carers.
  • Confidence to work as part of a team and alone in the community.
  • Able to adapt to changing patient needs.
  • Able to remain calm and rational in times of crisis.
  • Able to question and clarify information.
  • Able to pass on knowledge to other team members.

Qualifications

  • NVQ 2 in Rehabilitation, Promoting Independence or Care or equivalent
  • Care Certificate or willingness and ability to undertake (applicable for all non‑registered support workers that provide health care)
  • Equivalent qualifications may be considered if candidates meet all other elements of the person specification.
  • NVQ3 in Diagnostic and Therapeutic Care or equivalent
  • Additional training in rehabilitation or caring for the older adult.

Knowledge

  • Knowledge and understanding of community services.
  • Basic understanding of the Rehabilitation Assistant’s role.
  • Knowledge and understanding of the importance of rehabilitation and the promotion of independence.
  • Ability to question, clarify, knowledge of own limitations and importance of risk assessment.
  • Awareness of confidentiality requirements.
  • Knowledge of aids and equipment used to promote independence.
  • Knowledge of an MDT approach.
  • Knowledge of personal development and resources to support this (research articles, internet).

Personal Circumstances

  • Flexible and versatile approach to meet the needs of a 24/7 service.

Experience

  • Working in a health / social care related post, or worked in a voluntary or informal caring capacity with adults / older adults.
  • Previous experience of working in a team.
  • Work experience in a therapy / rehabilitation role.
  • Experience of working in a community setting.
  • Lone working experience.
  • Previous experience of working in a multi‑disciplinary team.

Other Requirements

  • Support equality and value diversity; genuine desire to work with older patients.
  • Able to travel independently of public transport.

Disclosure and Barring Service Check

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and a Disclosure and Barring Service check will be required. The cost of the DBS will be met by the successful candidate via salary deduction, at the standard or enhanced check rates.
