Job summary
ESSD Rehabilitation Assistant -25 hours. Based at Maryport Hospital, but covering Workington and Copeland areas.
The Early Supported Stroke Discharge (ESSD) service is excited to advertise a post for 25 permanent hours for a stroke-specific Rehabilitation Assistant to work within West Cumbria (Workington and Copeland localities). Our team is based at Maryport Hospital and you will be working with people in their own homes as well as across NHS and community sites, depending on the needs of the stroke survivor. The ESSD service plays a vital role in safe, supported and early discharge from the Hyper-acute Stroke Unit and the stroke wards at the Cumberland Infirmary and West Cumberland Hospital. We are a small team working with patients with mild-moderate levels of disability and a variety of needs. Our input is based around patient\’s own identified goals and is stimulating, varied and rewarding.
Main duties of the job
As a Rehabilitation Assistant in the team you will be key to the implementation of evidence based specialist treatment programs for patients following a stroke, who may have a range of impairments and complex health needs. The therapy plans often include supporting patients to practice their normal daily activities such as personal care, meal preparation and accessing their local community in order to achieve their rehabilitation goals. You will work closely with patients and their families and carers in the implementation of their care plans. You will be working with a full multidisciplinary team comprising of specialist Occupational Therapists, physiotherapists, Speech and Language Therapists and neuropsychologists.
The ESSD service works closely with hospital and community teams to provide a seamless transition of care from the acute hospital setting into the community. The post requires the ability to travel across the Workington and Copeland localities as required, as well as to work flexibility to meet service needs. You will provide specialist rehabilitation with patients following a stroke, in their own homes and other community and clinical settings under the direction and supervision of qualified therapists. The post is Monday to Friday but weekend working may be required as necessary.
