This Specialty Doctor post offers the opportunity to work as an integral member of the Bristol Care Home Interface Project, an interdisciplinary team providing proactive and holistic geriatric review to care home residents across BNSSG funded for 15 hours per week.
The post-holder will be part of a team comprised of physician associates, specialist clinical pharmacist, trainee Advanced Clinical Practitioner, GPwERs and geriatricians. The team aim to improve NHS support in care homes and strengthen links between care homes, community services and primary/secondary care.
We are looking for a GP with a specialist interest in frailty, or experienced frailty doctor, who can work as an independent practitioner when seeing patients in care homes and supervise more junior multidisciplinary team members. Please refer to the post-holder responsibilities within the job description.
Main duties of the jobThe Post-holder Responsibilities Will Include:
- Provision of expert clinical care to residents of care homes involved in the project, including detailed medication review and advance care planning.
- Liaison with next of kin for many residents to inform best interests decisions.
- Clinical assessment of residents with Parkinson's Disease and other Movement Disorders residing in care homes, and presentation of these patients at the Parkinson's MDT Hub meeting when needed.
- Clinical supervision for non-doctor medical professionals and doctors in training.
- Contribution to non-clinical tasks such as managing the team caseload.
- Contribution to governance and policy.
- Involvement in audit, research and service development, specifically data collection and analysis of project outputs.
- Onward planning for service development (including future funding applications) which supports the objectives of the broader organisation.
- Engagement with key stakeholders to achieve cross agency relationship building.
