An opportunity has arisen within the Assessment and Care Management Team, based within Brooksby Medical and Resource Centre, Largs. We are currently seeking a confident and professional Social Worker to join our team.
Reporting to the team manager, this role will include working with and assessing individuals over the age of 65 who are experiencing varying and complex support needs, and also adults over the age of 18 who are living with a physical disability.
You will be responsible for working within the legislative framework of safeguarding, undertaking assessments for Self-Directed Support, and the care management of vulnerable adults.
