Key Responsibilities Lead and manage a reablement service supporting people at home.
Focus on promoting independence and recovery after illness or hospital stay.
Oversee care staff, assessments, and care planning.
Work alongside health professionals to reduce long-term care needs.
Ensure safe, person-centred care and effective discharge planning.
The role includes peripatetic responsibilities, providing operational support across Care at Home teams as required, including assisting with absence management processes, undertaking home care reviews, and carrying out management investigations.
