Overview
Primary Care Paramedic Triage, in Clinic and Home visits.
Location: Croydon, South West London.
Salary: £55K‑£65K FtE annually, depending on experience and qualification.
Responsibilities
- Collaborate closely with GPs and other Healthcare Professionals to conduct patient assessments, assist with care plans, make referrals to community teams, update records and ensure patient safety.
- Administer vaccinations and manage patients within the appropriate care setting.
- Work as a First Contact Practitioner (FCP); you will be supported to register if not already.
- Provide acute and chronic disease management through home visiting, care‑home ward rounds, one‑day clinics for acute conditions, triage, long‑term condition reviews, COPD/asthma reviews, ECGs, BP checks, phlebotomy, learning‑disability and dementia health checks, and visit house‑bound or vulnerable patients.
Requirements
- HCPC registered Paramedic since 2021 or earlier.
- Interested in dealing with patient care in the community.
- Willingness to complete essential development and portfolio work.
- Interested in triage and able to manage patients remotely.
- Desirable: Urgent and Primary Care HEE level 6 education.
- Desirable: First Contact Practitioner level 7.
