The post holder is a Care Coordinator, who acts within their professional boundaries, working alongside the Practice multi-disciplinary team and within the Primary Care Networks. They work for the benefit of patients, providing and maintaining a high standard of care for patients' health needs. The post holder will be an integral part of the General Practice team, building relationships with other community providers, community services and working closely in partnership with other staff within the PCN and practices' other Social Prescribing Link Workers.
