We are looking for a motivated Advanced Nurse Practitioneror Advanced Clinical Practitioner to join our friendly and supportive team atMillwood Surgery. This is an excellent opportunity to work in a large,forward-thinking practice with a diverse patient population and strong GPsupport.

You will work alongside a team of six ANPs providinghigh-quality care to patients within our large practice population. Our diversepatient demographic offers a varied and rewarding scope of practice. The dutyGP is available throughout the day for clinical support when required.

Main duties of the job

You will provide advanced assessment, diagnosis, and management of patients, working autonomously with GP support. Applicants must be a registered healthcare professional (nurse and paramedic considered) with postgraduate-level training in advanced practice and ideally experience in a general practice setting. The role involves working across the four pillars of advanced practice clinical practice, leadership, education, and research contributing to patient care, service development, and professional growth.

About the Practice

Millwood Surgery is a well-established GP practice servingapproximately 19,800 registered patients. Our multidisciplinary team includes 8salaried GPs, Advanced Nurse Practitioners, a dedicated pharmacy team, and anin-house home visiting team, supported by experienced nurses, healthcareassistants, and administrative staff. We also work closely with colleagueswithin our Primary Care Network (PCN), providing additional support from awider multidisciplinary team to deliver high-quality, patient-centred care.Appointments are typically 15 minutes for ACP team, with protectedadministration time built into the schedule. Double appointments are availablefor patients presenting with more than one condition.

Job responsibilities

Job Purpose:

TheAdvanced Nurse Practitioner (ANP)/Advanced Clinical Practitioner (ACP) has been assessed as competent in practiceusingtheir advanced clinical knowledge and skills. They work autonomously andpart of a team to provide decisions in the assessment, diagnosis and treatmentof patients.

ANPs/ACPs inprimary care can develop close, long-term relationships with their patients andwork in partnership with them to achieve optimum health. They are autonomous inmaking decisions based on assessment, diagnosis and interpretation of testresults. They can independently prescribe appropriate medication, evaluate orrefer to other specialists if necessary.

ANPs/ACPs are registered nurses with advanced education and clinical trainingincluding non- medical prescribing. They must meet the NMC requirements foradvanced practice, which includes the 4 pillars clinical practice, leadership,education and research.

KeyResponsibilities:

The following are the core responsibilities of the AdvancedNurse Practitioner/Advanced Clinical Practitioner. There may be, on occasion, a requirement to carry out othertasks; this will be dependent upon factors such as workload and staffinglevels:

  • Develop, implement and embed health promotionand wellbeing programmes
  • Provide high-quality and advanced care forpatients of all ages with a range of conditions, including acute and chronicillnesses. They perform physical exams, take medical histories, and orderdiagnostic tests. They also develop and implement care plans, prescribemedications, and manage ongoing care via face to face, video or telephoneconsultations.
  • Make clinical decisions based on diagnosticresults and adjust treatment plans as needed.They must demonstrate safe clinical decision-making and expert care. Andwork within own scope of practice.
  • Work with physicians, specialists, and otherhealthcare professionals to provide patient-centered care. Provide clinicaltriage of patients to appropriate services and healthcare professionals.
  • Educate patients and their families abouthealthy lifestyle choices, disease prevention and self-management techniques.
  • Stay up to date with the latest evidence andstandards of care through ongoing education and research.
  • Mentor less experienced nurses and staff.
  • Support the Practices future development andshare the Practices values.
  • Manage patients presenting with a range of acuteand chronic medical conditions, providing subject matter expert advice
  • Identify, manage and support patients at risk ofdeveloping long-term conditions, preventing adverse effects to the patientshealth
  • Provide wound care where appropriate
  • Provide specialist clinics such as contraceptionwhere appropriate
  • Undertake the collection of pathologicalspecimens
  • Request pathology services as necessary
  • Process and interpret pathology and other testresults as required
  • Maintain accurate clinical records inconjunction with extant legislation
  • Ensure SNOMED CT codes are usedeffectively
  • Be responsible for the implementation of aneffective immunisation programme
  • Chaperone patients where necessary
  • Prioritise health issues and interveneappropriately
  • Support the team in dealing with clinicalemergencies
  • Recognise, assess and refer patients presentingwith mental health needs
  • Be an extended and supplementary prescriber,adhering to extant guidance
  • Support patients in the use of their prescribedmedicines or over the counter medicines (within own scope of practice),reviewing annually as required
  • Contribute to practice targets (QOF, etc.),complying with local and regional guidance
  • Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
  • Delegate clinical responsibilities appropriately(ensuring safe practice and that the task is within the scope of practice of the individual)
  • Support the clinical team with all safeguardingmatters in accordance with local and national policies
  • Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriatelyProvide chronic disease clinics, delivering patient care asnecessary, referring patients to secondary/specialist care as required
  • Deliver opportunistic health promotion whereappropriate

Secondary Key Responsibilities:

In addition to the primary responsibilities, the AdvancedNurse Practitioner/Advanced Clinical Practitioner may be requested to:

  • Act as the audit lead, effectively utilising theaudit cycle
  • Act as the IPC lead for the practice, ensuringcompliance, undertaking audit as necessary
  • Monitor and ensure the safe storage, rotationand disposal of medicaments
  • Participate in local initiatives to enhanceservice delivery and patient care
  • Support and participate in shared learningwithin the organisation
  • Continually review clinical practices,responding to national policies and initiatives where appropriate
  • Participate in the review of significant andnear-miss events applying a structured approach i.e., root cause analysis(RCA)
  • Develop practice administrative and clinicalprotocols in line with the needs of the patient and current legislation

Experience

  • Experience of practice within the four pillars
  • Job plan that demonstrates advanced nursing practice and has equity with peers working at this level
  • Experience of prescribing and undertaking medication reviews

  • Wound care/removal of sutures and staples
  • Understand the importance of evidence-based practice

Qualifications

  • Registered Nurse with Nursing and Midwifery Council
  • Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice or Post graduate diploma or degree for Advanced Practice Qualification up to December 2020
  • Qualified Independent Nurse Prescriber on the NMC register
  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
  • Meets the standards for registered ANP working at advanced level
  • Qualified triage nurse
  • Teaching qualification
  • ALS and PALS
