The NNUH Cancer Services team is growing its Information and Support Services with additional Cancer Care Navigator posts. As a Cancer Care Navigator, you will be uniquely positioned to direct newly diagnosed cancer patients through a Holistic Needs Assessment (HNA) process, providing information and support and signposting to relevant services. This enables patients and their carers to navigate the health and social care system and promote self-care. The team works closely with the body site clinical teams, local charities and inpatient care areas.

Key aspects of the role involve daily and direct (e.g., telephone and face‐to‐face) communication with patients, relatives, carers, and other health and social care professionals. This may be through telephone contact to undertake an HNA; manning the NNUH Information Pod or Cromer Centre for drop‐in advice and information; through holding and supporting outreach services in the community; holding Health and Wellbeing events to support self‐management; visiting patients within clinical areas. The information and nature of communication is sensitive. Communication in this context requires a high degree of empathy, understanding, diplomacy, confidentiality and integrity.

Responsibilities

You will be required to work from the Rouen road office and rotate through the information and support services at NNUH and Cromer. You will be expected to be able to talk to patients and carers confidently in clinical settings, and on the telephone.

Qualifications

  • Ability to deal with complex and difficult emotional situations
  • Awareness of the health and social care environment
  • Good IT skills that can cross different NHS IT systems
  • Effective organisational skills
  • Good communication skills

You will be a self‐motivated and enthusiastic individual with experience and understanding of the health and social care environment. A previous experience in a clinical role or administrative role with transferable skills will be considered.
