Reablement Assistant Practitioner 8949 | Permanent contract | £31,537 to £33,699 per annum (Scale G) | 37 hours per week | Grays Fair Court, Norwich

Would you like to work in a Housing with Care setting, supporting people to live independently with dignity and choice, then this role could be perfect for you.

We have a vacancy at Grays Fair Court, a Housing with Care site in Norwich, where you’ll work as part of a dedicated team alongside Operational Managers and support workers, helping individuals maintain their independence through personalised care and support planning.

Unlike our community-based roles, this position is based entirely within the Housing with Care environment. Instead of focusing on reablement care, you will be supporting individuals who live within the Housing with Care scheme and have been assessed under the Care Act as requiring ongoing domiciliary support.

As a Reablement Assistant Practitioner (RAP), you will be responsible for reviewing Housing with Care support plans, assessing individuals’ daily needs and developing personalised plans that reflect their preferences and goals. You will also complete Care Act Assessments when there has been a change in need or when long-term care is being provided. You’ll work closely with Operational Managers and other professionals in Adult Social Care to deliver high-quality, person-centred support.

You will need to have experience of working with people and be able to communicate clearly and appropriately with members of the public, whilst exercising judgement and using your initiative to plan and prioritise your work. You will also need knowledge of current policy, practice and legislation within Health and Social Care.

Full training will be provided to ensure you are confident and competent in delivering this vital role within our Housing with Care service.
