Overview
In this role you support children, young people and foster carers within a trauma-informed, relationship-based social care model. You provide specialist clinical advice to social workers and carers and engage in direct work when needed to drive sustained change. You work across the county as part of a multidisciplinary fostering team to improve outcomes. This is a meaningful opportunity to shape care plans and support services across the county.

Responsibilities
- Provide specialist clinical advice and consultation to social workers and foster carers
- Deliver direct work with children or young people when intervention is required
- Support foster carers transitioning from residential to family-based care and help devise individualized support plans
- Collaborate with social work teams and support services in a trauma-informed, integrated service
- Contribute to planning and delivering care that improves outcomes for children, young people and carers
