We are looking for a practitioner to join The Northern Learning Disabilities Team based in North Norfolk. The service provides an integrated Health and Social Care approach to assessment and support in the community across North Norfolk and Broadland.

Key responsibilities include:

  • Conducting assessments and supporting people with Learning Disabilities in the community.
  • Working under pressure, prioritising workload and delivering high quality service.
  • Communicating effectively with service users, families and multi‑agency partners.
  • Record‑keeping, data entry and maintaining patient information accurately.
  • Working within the multi‑disciplinary team to plan and review support strategies.

Essential qualifications and experience:

  • Relevant qualification in social care, health or a related field.
  • Experience working with adults with Learning Disabilities preferred.
  • Excellent communication, organisational and keyboard skills.
  • Commitment to excellent customer service and multi‑agency working.
  • Ability to build strong partnerships with service users and their families.
