We are seeking an enthusiastic Assistant Practitioner to join our team, supporting people in the community following their discharge from hospital. You will visit individuals in their own homes, residential and nursing settings, and hospital wards to help them achieve their desired outcomes and promote independence through a strengths-based approach.
Responsibilities
- Provide community-based support after hospital discharge, visiting individuals in their homes, residential and nursing settings, and hospital wards.
- Promote independence through a strengths-based approach and help individuals achieve their outcomes.
- Maintain excellent communication, record keeping, and keyboard skills; manage assessments in a timely manner due to the fast-paced nature of the work.
- Work within the West Discharge Follow On Team on a full-time basis, adopting a hybrid working model (office-based and remote).
- Hold a full driving licence and have access to a vehicle; travel across West Norfolk and occasionally other areas of Norfolk as required.
- Collaborate as a team player while also being confident to work independently and make appropriate decisions.

- Engage with the Norfolk County Council transition to a Human Learning Systems approach, focusing on meaningful local connections and community support.
Qualifications
- Essential: experience in social care or a willingness to join via the supported Assistant Practitioner programme.

- Ability to work independently and as part of a collaborative team.
- Full driving licence and access to a vehicle required; willingness to travel across Norfolk as needed.
