Norfolk Community Health and Care NHS Trust is recruiting for the Urgent Community Response Therapy Team covering Norwich. We are looking for enthusiastic and motivated individuals to join us in providing high-quality therapy services as a Community Therapy Assistant Practitioner.
The role involves:
- Providing support to the therapy team covering admission avoidance, urgent therapy referrals, discharge to assess, and NEAT services.
- Assessing a variety of patients with physical needs and providing a holistic approach to maintaining their independence in their own homes.
- Working across a 7-day week.
Main duties include:
- Contributing to the delivery of therapy care pathways for patients with long-term conditions, palliative care, and rehabilitation needs.
- Preventing unnecessary admissions to hospital.
- Supporting early discharge from hospital.
- Collaborating with health care professionals and agencies to provide a seamless service.
Job responsibilities:
- Assessing patients with straightforward therapy requirements based on predetermined department protocols.
- Preparing for, carrying out, and monitoring assessments and treatments in specified clinical areas.
- Applying competency-based treatment techniques under supervision.
- Observing patients in their homes and advising on equipment or methods to ease daily tasks.
- Assisting patients with personal care as appropriate.
- Modifying and progressing interventions using clinical reasoning.
- Monitoring patients and alerting qualified practitioners of unexpected changes.
- Planning and prioritising visits to delegated patients and documenting on SystmOne.
- Establishing therapeutic relationships with patients and carers.
- Liaising with the wider Urgent Community Response Multi-disciplinary Team.
Person Specification:
- Qualifications: NVQ 3 in health and social care or equivalent knowledge and skills from a therapy role.
- Experience of working in a therapy team or community therapy setting.
- Ability to manage stressful situations and work flexibly.
- Professional appearance and reliability.
Other requirements:
- Must hold a full and valid driving licence and have access to a vehicle.
- This is a 7-day service; some weekend working will be expected.
