Norfolk Community Health and Care NHS Trust is hiring Clinical Support Workers within the state-of-the-art Willow Unit at Norwich Community Hospital, which provides a supportive environment for 48 patients. The unit enables patients to make steady improvements and gain independence as they prepare to return to the community. Patients are supported to play an active role in their own care and feel confident about performing daily activities when they leave.

Main duties of the job:

  • Utilising the latest therapy and rehabilitation technology and practices, you will be a part of this team and will play a pivotal role in supporting patients back into the community.
  • Work as part of the team to deliver care in a reablement setting.
  • Support an environment where all team members are responsible for a reablement ethos using a positive risk-taking approach for patients, their carers and families.
  • Support patients being as independent as possible with their washing/dressing and nutritional needs.
  • Work as part of a multidisciplinary team within a quality framework to ensure that the highest standards of care are met.
  • Support and deliver innovation within discharge practice and pathways.
  • Work with system partners including the voluntary sector to ensure that patients experience a seamless transfer of care both into and onward from the unit.
  • Support the delivery and co-ordination of care to a named group of patients.
  • Work proactively within the team to achieve unit and trust goals and objectives.
  • Work with and promote recognised policies, protocols, and guidelines.

Job responsibilities:

  • Carry out the instructions accurately and promptly of the reablement ward team who are professionally accountable for the delivery of care.
  • Work within an agreed reablement competency framework to support patients with their stated goals and within their functional capabilities.
  • Report to registered staff any patient that gives cause for concern or any observed changes in a patient's clinical condition.
  • Support patients' emotional well-being by promoting independence on a range of activities of daily living.
  • Accurately document all care delivered to patients, including recording documentation electronically on trust systems while maintaining confidentiality.
  • Respond appropriately to clinical emergencies to ensure the patient receives appropriate care.
  • Be aware of and adhere to all trust policies/procedures/nursing guidelines when undertaking any activities.
  • Make patients welcome to the clinical area and participate in the admission process under the supervision of registered staff.
  • Provide and deliver a high standard of individualised personal care (e.g., washing/dressing and nutritional needs) with a reablement ethos.
  • Keep the unit clean and tidy maintaining a safe environment.
  • Adhere to trust infection control procedures.
  • Chaperone/escort patients to other clinical departments and/or NHS Units under the direction of the registered nurse.
  • Support relatives under the guidance of the Multidisciplinary Team (MDT).
  • Assist with the discharge process for patients by the MDT.

Person Specification:

  • Education to GCSE level Grade 5 in English and Mathematics or equivalent.
  • Care Certificate and/or equivalent health care experience.
  • City and Guilds NVQ Level 3 in Care/Health and Social Care.

Skills, Abilities and Knowledge:

  • Good organisational skills.
  • Basic I.T. and computer skills.
  • Good interpersonal and communication skills.
  • Good numeracy and literacy skills.

Other:

  • Able to make own travelling arrangements.
  • May be required to work in other community hospitals within our trust in order to meet the needs of the service.
  • Ability to work 12.5 hour shift day/night patterns.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

Salary: £24,937 to £26,598 a year pro rata per annum.
