An exciting Case Manager permanent part time position has arisen for a registered nurse to join our small and friendly team based at Attleborough Health Centre. The Frail and Elderly Team provides Case Management support to patients at home with long term conditions who are at high risk of acute admission due to health and complex personal needs.

Main duties of the job:

  • Assess and engage patients in self-management of their conditions, linking with key stakeholders to achieve a positive outcome for the patient.
  • Attend Monthly MDT meetings at the local GP surgeries.
  • Assess, plan, implement and evaluate all aspects of patient care, and develop treatment/care plans that may need to be delivered from a range of options.
  • Following a holistic assessment, devise an individualised care programme for each patient, modifying it as required.
  • Be aware of Assistive Technology and utilise where required.
  • Assess patients' holistic needs, communicating complex and sensitive information to patients and carers as to their assessment, diagnosis, prognosis and treatment plan.
  • Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.
  • Contribute to Personal Health Plans as required.
  • Undertake risk assessments and act upon them appropriately.
  • Participate in multi-disciplinary/multi-agency meetings as appropriate.
  • Ensure activity data and clinical information is recorded on SystmOne and completed on a daily basis.
  • Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advanced sensory skills, manual and mobilising skills.
  • Order equipment and supplies via the electronic ordering systems used within the team.
  • Ensure informed consent is obtained prior to initiating interventions.
  • Utilise universal precautions and adhere to infection control policies.

Person Specification:

  • Able to communicate effectively using the English language in both verbal and written form.
  • Must hold full and valid driving licence and have access to a vehicle.
  • Knowledge of additional specialist areas through in-house training and short courses.
  • Highly motivated and able to provide high standards of care.
  • Tact and diplomacy, empathy and sensitivity.
  • Ability to use own initiative and good observational and reporting skills.
  • Ability to manage stressful situations and concentrate when undertaking patient care and inputting data/patient records.
  • Good interpersonal skills and a team player.
  • Able to kneel, bend and stoop, and work in cramped environments.
  • Able to manoeuvre limbs of around 5-6 kg and patients using handling aids.
  • Have basic IT/standard keyboard skills, cannulation / IV Therapy administration, and awareness of Assistive Technology.

Experience:

  • Evidence of continuous professional development.
  • Evidence of working at relevant level and experience of working at a higher level.

Qualifications:

  • Assessor/mentorship preparation; ENB 998 or equivalent.
  • Current NMC registration.
  • Long Term Conditions module.
  • Independent/supplementary prescribing qualification.
  • Clinical assessment skills.
