Swaffham Community Hospital aims to deliver high quality, safe and person centred care. Our 18 bedded unit provides rehabilitation to a wide group of patients with varying complexities of needs. Our patients come to us following medical or surgical treatment or if appropriate, directly from their own homes. We also provide palliative and end of life care. Opportunities exist for further training and development within the role. You will be joining an enthusiastic, supportive, friendly multidisciplinary team. The post involves rotation between day/night duties. Our focus is on continually improving the quality of care we offer to local people and on improving access to that care, helping people to move seamlessly from one service to another.

Main duties of the job:
- Implement care for patients as defined by the individualised care plan.
- Provide a high standard of direct care for patients.
- Work in a collaborative manner with all other staff working within the unit.
- Establish a good relationship with the patients and provide an appropriate support mechanism so the patients are able to express their needs.
- Ensure the patients are involved in the decision making process, communicate all relevant information regarding the needs of the patients, family and friends.
- Document and sign all care interventions provided.
- Be involved in the induction and mentoring of new staff where applicable.
- Ensure effective and efficient use of all ward/department resources.
- Maintain a positive approach towards achievement within the team.
- Ensure adequate provision and maintenance of equipment.
- Assist with the induction of new staff and students, as a mentor.
- Act as a role model giving guidance and support to junior staff and students.
- Participate in problem solving that is related to your work and organisation.
- Ensure that patients and carers are aware of their current and ongoing health needs.

Job responsibilities:
- Accurately and promptly carry out the instructions of the Registered Nurse (RN) / Registered Nursing Associate (RNA) who is professionally accountable for the delivery of care.
- Carry out duties as specified within the trusts Approved Core Task List for Clinical Support Worker (working within assessed competence).
- Report to registered staff any patient that gives cause for concern or any observed changes in a patient's clinical condition.
- Support patients' emotional well-being by supporting positive conversations, actively encouraging participation in ward-based activities.
- Accurately document all care delivered to patients, including recording documentation electronically on trust systems maintaining confidentiality at all times.
- Respond appropriately to clinical emergencies to ensure the patient receives appropriate care.
- Be aware of role in emergency situations and what actions should be undertaken.
- Adhere to all trust policies / procedures / nursing guidelines when undertaking any activities.
- Make patients welcome to the clinical area and participate in the admission process under the supervision of registered staff.
- Provide and deliver a high standard of individualised personal nursing care.
- Keep the clinical area clean and tidy maintaining a safe environment.
- Adhere to trust infection control procedures.
- Assist in the preparation of patients and equipment for treatment, investigations or procedures as directed by registered staff.
- Chaperone/escort patients to other clinical departments and/or NHS Units under the direction of registered staff.
- Support relatives under the guidance of registered staff.
- Assist with the discharge process for patients directed by registered staff and/or member of Multidisciplinary Team (MDT).
Person Specification:
- Qualifications: Education to GCSE level Grade 5 in English and Mathematics or equivalent, Care Certificate and/or equivalent health care experience, City and Guilds NVQ Level 3 in care/health and social care.
- Experience: Some previous experience in a nursing/care assistant role.
- Skills, Abilities and Knowledge: Good organisational skills, Basic IT and computer skills, Good interpersonal and communication skills, Good numeracy and literacy skills.
- Communication: Clear oral communication, Basic computer skills.
- Personal and People Development: Caring personality, Sensitive and tolerant, Resilient.
- Personal Attributes/Behaviours: Good interpersonal skills, Responds positively to change, Flexible, Ability to work as part of a multi-disciplinary team.
- Other: Able to make own travelling arrangements, Ability to work 12.5 hour shift day/night pattern, May be required to work in other community hospitals within our Trust in order to meet the needs of the service.
