NCH&C is hiring Clinical Support Workers within the state‑of‑the‑art Willow Unit at Norwich Community Hospital, a supportive environment for 48 patients. The unit enables patients to make steady, step‑by‑step improvements, gain independence, prepare to return to the community, and perform daily activities with confidence.

Healthcare is delivered by a multidisciplinary team of clinical professionals dedicated to patient recovery and well‑being.

Main duties of the job

Utilising the latest therapy and rehabilitation technology and practices, you will be part of a team that supports patients back into the community and helps create a reablement culture within this new unit.

Work as part of the team to deliver care in a reablement setting.

Support an environment where all team members uphold a reablement ethos, using a positive risk‑taking approach to benefit patients, their carers and families.

Help patients become as independent as possible with their washing/dressing and nutritional needs.

Participate in a multidisciplinary team within a quality framework to ensure the highest standards of care are met.

Support, deliver and innovate within discharge practice and pathways.

Collaborate with system partners, including the voluntary sector, to ensure patients experience a seamless transfer of care both into and onward from the unit.

Coordinate care for a named group of patients.

Work proactively within the team to achieve unit and trust goals and objectives.

Promote and adhere to recognised policies, protocols and guidelines.

Job responsibilities

Carry out the instructions accurately and promptly from the reablement ward team who are professionally accountable for the delivery of care.

Work within an agreed reablement competency framework to support patients with their goals and within their functional capabilities.

Report to registered staff any patient that causes concern or any observed changes in a patient’s clinical condition.

Support patients’ emotional well‑being by encouraging positive conversations and promoting independence in daily activities.

Accurately document all care delivered, including electronic recording on trust systems while maintaining confidentiality.

Respond appropriately to clinical emergencies to ensure the patient receives suitable care, understanding the role and necessary actions in emergency situations.

Adhere to all trust policies, procedures, nursing guidelines when undertaking activities.

Make patients welcome to the clinical area and participate in the admission process under supervision of registered staff.

Provide a high standard of individualised personal care (e.g., washing/dressing, nutritional needs) with a reablement ethos.

Keep the unit clean, tidy and a safe environment.

Follow trust infection control procedures.

Chaperone/escort patients to other clinical departments or NHS units under direction of the registered nurse.

Support relatives under guidance of the multidisciplinary team (MDT).

Assist with the discharge process for patients by the MDT.

Person Specification

Qualifications

  • Education to GCSE level Grade 5 in English and Mathematics or equivalent.
  • Care Certificate and/or equivalent health care experience.
  • City and Guilds NVQ Level 3 in Care/Health and Social Care.
