Key Responsibilities:
- Conduct comprehensive assessments of individuals' care and support needs.
- Develop, implement, and review individualized care and support plans ensuring they meet the highest standards of quality and compliance.
- Safeguard vulnerable adults and children in line with local policies and national regulations.
- Coordinate and liaise with health and social care professionals, voluntary organizations, and other stakeholders.
- Advocate for service users' rights and promote social inclusion and wellbeing.
- Maintain accurate, confidential case records and reports.
- Participate in continuous professional development and supervision sessions.
