Hours: School-friendly hours, evening shifts, weekend work, or a flexible role around other commitments.
Weekend Work: Weekend availability required

About the Role
As an Outreach Support Worker, you will provide person-centred support to individuals, helping them maintain their independence and achieve their goals. Every day is different, and you will play a vital role in improving the quality of life of the people we support. You will be supporting individuals in their own homes and within the community.
Responsibilities:
- Providing support in individuals' own homes and within the community.
- Assisting with daily living activities.
- Promoting independence, choice, and dignity.
- Supporting individuals to attend appointments and social activities.
- Encouraging engagement in hobbies, interests, and community involvement.
- Maintaining accurate records and following care plans.
What We Are Looking For
- A caring and positive attitude.
- Good communication and interpersonal skills.
- Reliability and flexibility.
- Experience in care is beneficial but not essential as full training will be provided.
- Ability to work independently and as part of a team.
Essential Requirements:
- Full UK Driving Licence
- Access to a reliable vehicle
- Right to work in the UK
